[Fredslist] Medical Assistance at Home

Dear all,

I have a very good client of mine that needs assistance for his dad.

His dad is currently at Cold Spring Hills in Syosset in Hospice care. The family ultimately would like to bring him home to his house in Astoria Queens.  The family is ready, willing and able to make any necessary adjustments to the home to make this happen.  They feel they would need 100 hours a week of assistance, 20 hours of skilled nursing (an LPN) and 80 hours per week of aides.  Dad currently has a trach and PEG.

Please let me know if you know of any resources this family can turn to help them find the care that they need.

Simply email me the information and let me know if you have or have not had personal experience with your recommendation.
