Ok Gotham – we need some Gotham magic -

My client has a small business in which she provides health insurance for her 4 employees.  Apparently their  health insurance was canceled without a notification to their agent, my client, or the employees covered by the insurance. In fact they only found out about it when an employee went to the ER and was informed his insurance was no good.  They have paid every premium on time.  The insurer claims that the issue is a survey was mailed to their agent in May and July. When it was not returned, the policy was canceled. It looks like reinstatement retroactive to Sept 1 has been denied.  We think the real issue is that the policy only covers 4 people – even so – they need a new insurance policy asap.
